Stage V of the nursing process

At the 5th stage of the nursing process (SP) there is a final assessment of the nurse's activities based on the achieved results of patient care.

 

Outcomes are continuously evaluated throughout the nursing process. Starting with the patient's identified problems, the nurse analyzes her activities and the patient's response to nursing interventions.

 

The areas in which the nurse's performance is assessed at all stages of the SP:

  • the reality of achieving the goal of care;
  • patient response to all related nursing interventions;
  • analysis of problems arising in the process of care and search for their solutions.

 

When the goal is achieved and the patient's problems are positively resolved, the nurse makes an appropriate mark in the care plan.

 

In the absence of a result, the problems are reassessed and the reasons for the ineffectiveness of care are established. As a result of the analysis of nursing activities, it is possible to change the goals and tactics of care . Nursing interventions adjusted in the plan

 

It is important to remember that the assessment of the activities of the nurse and the correction of further care is carried out together with the patient.
